What we collect, and why.

This covers the enquiry form. It is written to be read, not to be survived.

What the enquiry form collects

Your name and how to reach you. What you told us you are looking for, and who it is for. Whether you need an interpreter or help completing the form. Practical details you chose to give: date of birth, address, Medicare number, availability. Everything beyond a name and one contact is optional.

Why we ask for an address

If someone becomes unsafe during an online session we need to know where they are so help can be sent. That is the only reason it is asked for.

What we do not collect here

No medical history, no medication list, no diagnosis and no clinical detail. The form is not the place for that and nothing on it asks for it.

Where it goes

One email to the practice, read by the clinician the enquiry is routed to and by practice administration. It is not shared with anyone outside the practice, not used for marketing, and not sold. If we need to refer you elsewhere we will ask you first.

How long we keep it

If you go on to become a client, it moves into your clinical record and is kept as long as the law requires. If you do not, the enquiry is deleted within twelve months.

Asking to see it or have it removed

Write to admin@anoshetherapies.com.au and we will tell you what we hold and delete it if you ask, unless a clinical record obliges us to keep it.

This is a collection statement for our enquiry form, not our full privacy policy. If you are already a client, the handling of your clinical record is governed separately and your clinician can walk you through it.
